Dear Parents & Guardians,

Please watch for a very specific consent form from your child’s school. This will be titled ”First Aid & Basic Health Care Consent.” This form is in response to Public Chapter 1061, also known as the Families’ Rights and Responsibilities Act (FRRA).

This new law requires parent permission for a student to be seen in a nurse’s office or given basic health care/first aid via school personnel. The consent form will allow you as the parent/guardian to indicate if you DO or DO NOT give permission for your student to receive basic minor care from the school nurse and/or school personnel. Please note, if the nurse or school personnel indicates that a student needs anything above minor care, school personnel and/or the nurse will always seek further consultation with the parent/guardian. This consent form is to indicate that minor care can be given (i.e. band aids, gauze, ice pack, etc.). If you do not wish to give consent, your child will only be treated in an emergency situation, and you will be called to care for your child any time your child requests or needs to use the Nurse’s Office.

The form will be sent out multiple ways: on ParentSquare and in paper form, so be on the lookout.

Please review, mark your preference regarding consent and return the consent form as soon as possible so there is no delay in care that is needed for your child.
